Short answer
Medicare pays hospices a daily rate to provide the services in each patient's plan of care. Federal rules list homemaker services, which 'may include assistance in maintenance of a safe and healthy environment.' A hospice can decide to arrange and pay for mattress sanitation when the team thinks it's needed. That's the agency's call. Medicare doesn't pay us directly, and we don't bill Medicare.
Key terms used on this page
- Hospice per-diem:
- The fixed daily rate Medicare pays a hospice, which covers the services in the patient's plan of care.
- Safe and sanitary environment:
- Federal hospice rules' phrase for the household tasks aides may do in areas the patient uses, such as changing linens and light cleaning.
Hospice teams sometimes ask whether mattress sanitation can be part of the plan of care. Here's how the federal framework works, without overstating it.

How hospice is paid
The Medicare Benefit Policy Manual says hospices are paid a per-diem rate based on days and level of care, and the hospice must provide the services in the patient's plan of care.
Homemaker services
42 CFR 418.202 lists homemaker services among covered hospice services: they "may include assistance in maintenance of a safe and healthy environment and services to enable the individual to carry out the treatment plan." Aides may also do household tasks to keep a sanitary environment in areas the patient uses.
What that means in practice
A hospice can choose to arrange and pay for a mattress visit when its team thinks it's needed, for example persistent urine odor in the patient's bed. That decision and the payment belong to the agency. We don't bill Medicare and aren't enrolled in any government program.

Sleep Sanitation's view
This section is our view. Agencies that group visits with us for route efficiency can ask about agency rates. We carry a certificate of insurance, available on request, and run background checks under our hiring policy, including a national sex offender registry check. Sleep Sanitation serves Omaha, Lincoln and surrounding towns. Every visit includes dry vapor steam, HEPA vacuuming, UV-C light treatment, the built-in bed mite sensor on our JIGOO UV-C mattress vacuum and a moisture check before we leave. Enzyme treatment is used on urine and other biological jobs, and ordinary accident treatment is included in the $249 first-mattress price. Severe or biohazard contamination carries a surcharge, and we tell you before any work starts. Call or text (402) 512-5658, or email info@sleepsanitation.com.
