Short answer
Smoke smell lingers because smoke residue sinks into fabric and filling and keeps re-releasing into the air. Researchers found new cotton pillows placed in former smokers' homes absorbed nicotine into the case, fabric and filling within three weeks. A follow-up study lists mattresses among the materials that hold residue and found repeated cleaning works best. HEPA vacuuming, dry vapor steam and UV-C, repeated as needed, are how we remove it.

Cigarette, cigar, cannabis or wildfire smoke: if it was in the room, it's in the bed. Searches for "how to get smoke smell out of mattress" come from new homeowners, people who bought a used mattress and people who just quit.
Why smoke smell won't leave a mattress
Smoke leaves a residue that settles on and into soft materials and keeps giving off odor after the smoking stops. In one study, researchers placed new organic cotton pillows in the homes of former smokers; within three weeks the pillows had absorbed nicotine in the case, the fabric and the cotton filling (Matt et al., 2019). A mattress has far more fabric and fill than a pillow.
Why one cleaning isn't always enough
A study of cleaning in smokers' former apartments lists mattresses among the "reservoirs" that store smoke residue, and concludes that bigger, longer-lasting reductions "may require repeated and frequent cleaning" (Matt et al., 2021). That's why we treat heavy smoke odor as a plan, not a single spray.
What to do
- Clean the room too. Wash bedding, curtains and pillows, and run the HVAC fan with a fresh filter, so the room doesn't re-scent the mattress.
- Skip the perfume. Fresheners layer scent over smoke (fragrance masking).
- Professional cleaning. HEPA vacuuming captures the fine particles in the surface, low-moisture dry vapor steam cleans the cover and top layers, and UV-C light treatment finishes the surface.
- Repeat on schedule. Heavy smoke odor responds best to repeated cleanings; twice a year keeps it from settling back in.

How we get the smell out
This section is our view. Every Sleep Sanitation visit starts with a nose-and-eyes inspection to find where the odor actually lives, then HEPA vacuuming, enzyme treatment on organic odor, low-moisture dry vapor steam from our Italian-made Vapor Clean machines, UV-C light treatment and a moisture check before we leave. That sequence goes after the source of the smell instead of covering it with fragrance. The first mattress is $199 Fall 2026 special (regularly $249), any size. We recommend cleaning every mattress twice a year so odor never gets a head start. Not sure what your bed needs? Take the 60-second mattress quiz, or schedule your cleaning.
