The mattress item on a discharge checklist

By Matthew Brunken · Updated October 27, 2026 · 2 min read · Editorial standards

Short answer

Medicare's discharge planning checklist asks families to plan for equipment, help at home and household tasks like house cleaning. The bed the person will sleep in fits naturally on that list. Book a mattress visit ahead of the homecoming date, so the bed is cleaned, sanitized and moisture-checked, with a Service Summary for the family and care team.

Key terms used on this page

Discharge checklist:
A list patients and caregivers use to prepare to leave a hospital, nursing home or other care setting. Medicare publishes one (CMS Product No. 11376).
Care transition:
A move between care settings, such as coming home from a hospital, rehab or nursing home stay, or moving into assisted living.
Service Summary:
Sleep Sanitation's per-visit service record, called the Service Summary: mattress inspection, sanitation performed (mattress surface verified at 212°F or higher with a temperature gun, glide rate) and technician confirmation. The customer gets a copy.

Home readiness

The mattress line on a discharge checklist

  • Homecoming date known
  • Mattress visit booked for a day before homecoming
  • Oxygen, breathing sensitivities and enzyme allergy mentioned at booking
  • Rented hospital bed: check with the supplier first
  • Service Summary to the family, and to the care team with permission

Discharge planning covers a lot: follow-up care, medicines, equipment and who will help at home. The bed the person comes home to is easy to forget.

What the checklist covers

Medicare's discharge planning checklist is written "for patients and their caregivers preparing to leave a hospital, nursing home, or other care setting." It tells families to "Ask if you'll need medical equipment (like a walker), if it's covered by Medicare, and who will arrange for its delivery," and to ask whether they'll need help with tasks including "house cleaning."

Adding the mattress

Add one line under home readiness: "Mattress cleaned and sanitized before homecoming."

  1. Date set. As soon as the homecoming date is known, call or text (402) 512-5658.
  2. Book the visit for a day before the homecoming. Allow 1–2 hours per mattress.
  3. Mattress visit. HEPA vacuuming with UV-C, enzyme on any urine, low-moisture dry vapor steam with the surface verified at 212°F or higher with a temperature gun, and a moisture check before we leave.
  4. Homecoming. The family has a Service Summary, and a copy can go to the care team with permission.

Hospital beds

If a home hospital bed is arriving, we can clean the person's own mattress, and we service vinyl and coated hospital bed mattresses. We don't service powered air, low-air-loss or alternating-pressure mattresses. For a rented bed, check with the supplier first. See preparing the bed after a hospital stay.

Sleep Sanitation's view

This section is our view. Agencies that group visits by area are route-efficiency partners and get reduced agency rates: request agency rates. We carry a certificate of insurance, available on request, and run background checks under our hiring policy, including a national sex offender registry check. Sleep Sanitation serves Omaha, Lincoln and surrounding towns. Every visit includes dry vapor steam with the mattress surface verified at 212°F or higher with a temperature gun, HEPA vacuuming, UV-C light treatment and a moisture check before we leave. Allow 1–2 hours per mattress. Our technician removes the mattress protector during the visit and leaves a one-time supply of special detergent sheets for washing it. Enzyme treatment is used on urine. Every visit ends with a Service Summary, and a copy can go to the agency with the client's or family's permission. The first mattress is $199 Fall 2026 special (regularly $249); additional mattresses in the same visit are $199 for a king or California king, $179 for a full or queen and $149 for a twin or kids bed. Severe soiling carries a surcharge, disclosed before any work starts. Call or text (402) 512-5658, or email info@sleepsanitation.com.
